9. Select your date of birth from the calendar.

10. Select your sex from the dropdown list.

11. Please specify your address, state, district and PIN code for correspondence.

12. Enter your Mobile number (10 digits only).

13. Enter your valid E-Mail id.

14. Enter the Aadhar No./Voter ID No./Any other ID and Address proof No(Issued by Govt.)

15. Upload Photo - Acceptable Photo Size – 20 KB to 50 KB(in .jpg/.jpeg format only).

16. Upload Signature - Acceptable Sign Size – 10 KB to 30 KB(in .jpg/.jpeg format only).

17. Registration Fee is Rs 2500 /- (Registration Fee is not refundable once paid)

18. Payment Mode: Online (Net Banking / Debit Card / Credit Card)

DIPLOMA IN MEDICAL LABORATORY TECHNICIAN
COURSE DURATION : 2 YEARS
ELIGIBILITY CRITERIA : CANDIDATE SHOULD BE INTERMEDIATE (12TH) PASSED WITH SCIENCE SUBJECT BIOLOGY IS COMPULSORY.

DIPLOMA IN OPERATION THEATRE
COURSE DURATION : 2 YEARS
ELIGIBILITY CRITERIA : CANDIDATE SHOULD BE INTERMEDIATE (12TH) PASSED WITH SCIENCE SUBJECT BIOLOGY IS COMPULSORY.

CERTIFICATION IN DRESSER
COURSE DURATION: 1 YEAR
ELIGIBILITY CRITERIA : CANDIDATE SHOULD BE 10TH PASSED.
